Sagging Face & Jowls
When the lower face starts to feel heavier or less defined.
Facial sagging is often noticed first around the jawline: the contour becomes less crisp, the corners of the mouth may appear heavier, and early jowls begin to change the shape of the lower face. It is tempting to think of this as simply ‘loose skin’, but facial ageing is more complex.

Why Does the Face Begin to Sag?
Ageing affects several layers at the same time. Skin gradually loses collagen and elasticity; facial fat compartments change; retaining ligaments and deeper support structures evolve; and changes in bone support can alter how soft tissue sits on the face.
The result is not identical in everyone. One person may mainly have skin laxity, another may have volume redistribution, and another may have lower-face heaviness despite relatively good skin.
Jowls Are Usually a Result,
Not a Single Problem
Treating the jowl itself does not always address why it formed. Adding filler directly around a heavy lower face can sometimes increase bulk rather than improve definition. Assessment therefore includes the cheek, pre-jowl area, chin, jawline, skin quality and overall facial proportions.
What We Assess
We look at the degree and pattern of laxity, facial volume distribution, chin and jaw support, skin thickness and quality, lower-face width, previous fillers or threads, and how your face moves naturally.

Treatment Options
Collagen stimulation and structural support
Ellansé, Aesthefill or selected dermal fillers may be considered when loss of support contributes to the change in contour.
RF microneedling / EXION
RF microneedling may be used in suitable patients to support collagen remodelling and improve skin quality and firmness.
Thread lifting
Threads may be considered for selected patients with appropriate tissue characteristics and realistic expectations. They are not a substitute for surgical lifting when laxity is advanced.
Facial contouring
Improving chin, jawline or surrounding structural balance can sometimes make the lower face appear cleaner without trying to fill every visible fold.
When Non-Surgical Treatment Has Limits
Non-surgical treatments can improve selected features, but they cannot reproduce the degree of repositioning achievable with surgery. If the degree of laxity exceeds what injectable, energy-based or thread treatments can reasonably improve, this should be discussed openly.
Frequently Asked Questions
Can filler lift jowls?
In selected faces, strategic structural support may improve the appearance of the jawline. Simply adding filler around a jowl is not always appropriate.
Are threads suitable for everyone?
No. Suitability depends on tissue thickness, laxity, facial anatomy, previous treatments and the degree of change expected.
Can I prevent sagging?
Ageing cannot be stopped, but sun protection, maintaining skin health, avoiding major weight fluctuations and appropriate collagen-supporting treatments may help preserve skin and tissue quality.
The Aim Is a Cleaner Contour, Not an Over-Tightened Face
At Mira Clinic, the treatment plan is based on the pattern of ageing rather than a standard lifting package. Book a consultation with Dr. Joann for an individual facial assessment.
