Under-Eye Hollowness & Tired Eyes
The eye area needs diagnosis before it needs filler.
The eyes strongly influence whether a face appears rested, alert and youthful. Hollowness, shadowing, pigmentation, thin skin or puffiness can all create a tired appearance - and they do not share the same treatment.

Not Every Dark Circle Is a Tear Trough
Darkness beneath the eyes may come from anatomical shadow, pigmentation, visible blood vessels, thin skin, swelling, eye bags or a combination of factors. Filling a hollow will not correct every cause of dark circles.
Why Mid-Face Support Matters
The under-eye area cannot be assessed in isolation. Changes in the cheek and mid-face can alter the transition between the lower eyelid and cheek. In some patients, improving surrounding support is more appropriate than placing product directly beneath the eye.
What We Assess
We consider the depth and location of hollowness, eye bags, skin thickness, pigmentation, cheek support, facial anatomy, fluid retention tendency, previous filler and the relationship between the eye and surrounding structures.
Treatment Options May Include
Structural support
Selected filler or other structural approaches may be considered when cheek or surrounding support contributes to the under-eye shadow.
Direct under-eye treatment
Direct treatment may be appropriate for selected patients, but the under-eye is a technically sensitive area and is not automatically treated simply because a tear trough is visible.
Skin-quality treatment
When thin, crepey or poor-quality skin contributes to the appearance, regenerative or skin-quality approaches may be discussed depending on suitability.
Pigmentation and lifestyle factors
If pigmentation, allergy-related rubbing, sleep disturbance or other non-volume factors dominate, correcting volume alone is unlikely to solve the concern.
When Filler May Be the Wrong Choice
Prominent eye bags, significant fluid retention, certain anatomical patterns or existing filler can make additional under-eye filler inappropriate. In these cases, alternative strategies or referral for surgical assessment may be more sensible.
Frequently Asked Questions
Will tear-trough filler remove my dark circles?
Only if shadowing from hollowness is an important cause. Pigmentation, vascular colour, thin skin and eye bags require different considerations.
Why does my under-eye filler look puffy?
Several factors can contribute, including product characteristics, placement, anatomy and fluid attraction. Existing treatment should be assessed before more product is added.
Can I treat tired eyes without filler?
Yes, depending on the cause. Skin-quality treatments, management of pigmentation or surrounding facial support may be more appropriate in some patients.
Treat the Cause of the Tired Look
Book a consultation with Dr. Joann for an assessment of the under-eye area together with the surrounding face, rather than choosing a treatment based on the hollow alone.
